Another graphic design mistake?

If you've seen the controversy over the front page of a newspaper depicting Jesus smoking and drinking, and read the story that it was a legitimate mistake by a graphic designer, re; Malaysian Newspaper Front page shows Jesus smoking and Drinking. then you'll be interested to know another graphic designer has sent this picture into Mr Bagel. Apparently it was a mistake like the other one.

I've been trying to decipher it, the best I can make out is a condom and a nursery rhyme book? Can't see how that could be related??
